Downloads Documents to complete application A home study is the best in-depth way for us to get to know you! It is a great opportunity for you to become organized to prepare for this life journey. What exactly is a
home study? The State of Oklahoma (as well as others, dependent upon state law)
requires all prospective adoptive couples to complete a pre-placement home
study, which includes a home visit with the entire family, and individual visits
with all household members at either the home or agency office. The home study
must be updated every 12 months as long as the couple is actively planning to
adopt. The study will also require releases to be signed to perform criminal
investigations in the couple’s state of residence along with a Sexual Offenders
Registry check. The Department of Humans Services will review the Child Abuse
records fingerprints and history for referrals or complaints of all household
members.
It will require
that the couple or individual verify the status of certain documents. Copies of
the following documents will be required: (you may also download the
Required Documents.) - Marital status (marriage license, divorce decree, etc.) - Employment (a letter from employer that verifies salary and position) -
Income (tax returns for past 3 years)
- Access
to medical care (letter from insurance provider that the child will be covered) - Physical
health and history (documents included in application) - You will also need to have 3 letters of recommendation from individuals that have seen you in your home, interact with children, and who will give an objective opinion of you and your family.
Many of these items
are also required for the application, but submission of the documents with the
home study will satisfy this requirement.
